Sign our Liability Waiver Form, full instructions below for new flyers or existing flyers who need to complete a form. All existing clients who want to book a class, click here.

New Flyer: Sign our Waiver Form and create your account. . .

  1. Complete our Liability Waiver Form. This form will automatically create a Utah Flying Trapeze MindBody account with us. A Liability Waiver Form is mandatory for each individual participant so you may need to return to our Liability Waiver and complete multiple forms. Minors (17 years and younger) need help and permission from their Parent/Guardian’s to complete the Liability Waiver Form. *Each individual must have a unique email and password, combination. Creating accounts with the same email and password will cause an error in the system.
  2. After completing the online Liability Waiver Form, check your email to find the links and information to access our MindBody booking system. Use your email address and personal password to access your account.
  3. Navigate under the CLASSES tab to the date, day and time of the class you wish to attend.
  4. Click SIGN UP NOW and proceed to check out and pay for your class. You will know you have successfully signed up for class when you receive the automatic confirmation email.

Existing Flyer: Update your Waiver Form on your existing account. . .

(If you have already completed the waiver, go to Class Schedule to sign up for class)

  1. Go to our Liability Waiver Form.
  2. Select “Already in Our System”
  3. Type in your Frist and Last Name and email address you used to create your account.
  4. You may be asked additional questions to locate and verify your account with us.
  5. Enter your existing password or create a new password.
  6. Complete all the fields on the Liability Waiver Form and Sign.
  7. Check your email to find the links and information to access our MindBody booking system. Use your email address and personal password to access your account.
  8. Navigate under the CLASSES tab to the date, day and time of the class you wish to attend.
  9. Click SIGN UP NOW and proceed to check out and pay for your class. You will know you have successfully signed up for class when you receive the automatic confirmation email.

 Cancelations

Classes require a 4 person minimum to operate. We will cancel classes with less than 4 students. You should receive notice of the cancellation by way of an automatic email and the credit of the class will be returned to your account. Please ensure you have provided us with your current and best email for notifications on your account and bookings. Cancellations not made by the Utah Flying Trapeze require 48hr notice to receive account credit. Cancellations under 24hr are not eligible for a refund or account credit.
